*EDU 589 IS A MANDATORY CLASS EFFECTIVE FALL 2011, FOR STUDENTS BEGINNING FALL 2011 AND AFTER. IT IS ALSO MANDATORY IF EDU 515 HAS NOT BEEN TAKEN, PRIOR TO FALL 2011.

(If students were admitted prior to the approval of this checklist, with the consent of the advisor students may elect to switch to this checklist, 5 different courses on the checklist may have been taken prior to unconditional admission.)

Note: I understand that I cannot have taken any graduate courses required for completion of the Alternative A program prior to my being unconditionally admitted to the program. Taking any graduate courses prior to being admitted makes me ineligible for admission to the Alternative A program. I have enclosed a copy of either my ETS Praxis II Score Report or 32x19 or 4x12.
