When you’re away at college and not feeling well, it may not be possible to get a batch of Mom’s chicken soup or special home remedy, but our Health Services Center can make sure you get the treatment you need to start feeling your best again.
ASU’s Health Services Center for students is located on the ground floor of Simpson Hall. The Center’s staff includes a registered nurse director, staff registered nurse, staff licensed practical nurse, administrative secretary and University physician.
In addition to treatment for illness and injury, the Health Center offers preventative care such as vaccinations and promotes a number of wellness and health education activities on campus.
All enrolled ASU students are eligible for services at the Health Center and are covered by the Fixed Indemnity Accident and Sickness Insurance Plan.
All new students are required to submit a completed Health Evaluation Form to the Health Center. The Director of Health Services reserves the right to interpret the validity of all documents submitted for compliance. Documents must be legible to be processed. A hold will be placed on your student account until all requirements are complete.
Supplemental immunization records attached to this form must also have an authorizing signature, date and office stamp or address.
