Eligibility Requirement: Must be employed one year before plan is effective. No medical examination is required to enroll.

Benefits: Plan pays 60% of your basic monthly salary, up to $5,000 monthly after 180 days of non-work status.  Pays an additional 40% for Critical Disability, up to $5,000 monthly. 

Continuation of Long Term Disability Insurance: Plan will cease when no longer employed by ASU on a full-time basis. Termination of insurance does not affect benefits for a disability existing on that day.
