Optional Supplemental Coverage:
Eligibility employees may choose additional amounts in increments of $5,000, to a maximum of $100,000. Monthly cost per $1,000 of coverge is based on age. $20,000 in spouse/domestic partner coverage or $10,000 for each dependent child coverage may also be purchased. Dependent children eligibility - 15 days to 19 years or age, or 26 years of age if enrolled as a full-time student.
Reduction:
Upon the attainment of age 70 the amounts of life and accidental death and dismemberment insurance shall be reduced 50%. Upon retirement all insurance benefits shall terminate.
Beneficiary:
As in writing, on file with the policyholder.
